Requirements

• Master’s Degree from a school accredited by the Council on Social Work Education (CSWE)
• Current state licensure in Missouri (and/or Illinois, as we expand)
• Minimum of one year of social work experience in a healthcare setting (home health experience preferred)
• Strong analytical and interpersonal skills
• Ability to work independently in a field-based environment

Responsibilities

• Initial Assessments: Lead the evaluation of clients’ social and emotional needs and develop comprehensive Plans of Care (POC)
• Advocacy & Counseling: Counsel clients and families on coping with the social and emotional impacts of illness
• Supervision: Provide clinical oversight and co-signing for Bachelor’s level social workers (LSWs/BSWs)
• Community Connection: Identify and link clients with appropriate community resources to ensure their safety and well-being
• Care Coordination: Collaborate with physicians and the clinical manager to report changes in condition and ensure seamless care
