We are seeking a compassionate and skilled Hospice Medical Social Worker to join our dedicated team of professionals in providing high-quality end-of-life care to patients and families in their homes or hospice facilities. The ideal candidate will be an empathetic and culturally sensitive social worker with a strong understanding of hospice and palliative care, and a commitment to enhancing the quality of life for individuals facing terminal illness. As a Hospice Medical Social Worker, you will provide emotional support, counseling, and resources to patients and families, assist with advance care planning, and ensure that patients' social, emotional, and psychological needs are addressed throughout their end-of-life journey.
