About The Position

We are seeking a Medical Social Worker (MSW) to join our Walnut Creek team. This role is crucial in addressing the emotional, psychological, and social needs of our patients and their families, ensuring they receive comprehensive care and access to vital resources.

Requirements

• Certified Social Worker - Required
• 2+ years of MSW experience in Home Health or Hospice setting - Required
• Experience in Hospice or palliative care - Required
• Strong interpersonal and counseling skills.
• Valid Drive License and reliable transportation.

Responsibilities

• Assess patients’ and families’ psychosocial needs and develop care plans.
• Provide counseling and emotional support to patients and caregivers.
• Assist families in accessing community resources, financial aid, and legal support.
• Collaborate with the interdisciplinary hospice team to ensure holistic care.
• Maintain accurate and timely documentation in compliance with regulations.
