This Social Worker position will function as a medical case manager that is part of the Center for Autism Services and Transition (CAST) program, located at OSU Internal Medicine and Pediatrics at Hilliard. CAST provides adult care for patients diagnosed with autism spectrum disorders and support for those who are transitioning from pediatric to adult care.

Requirements

  • Minimum of a Bachelor’s Degree in Social Work.
  • State of Ohio licensure (LSW).
  • Experience coordinating available community resources and payer programs and limitations is required.
  • Flexible to meet the needs of the patients and providers.
  • Demonstrative effectiveness in conveying information both in writing and verbally.
  • High level of interpersonal skills to handle sensitive and confidential situations.
  • Position continually requires demonstrated poise, tact and diplomacy.

Responsibilities
