About The Position

Jefferson Healthcare is seeking a full-time Licensed Independent Clinical Social Worker to join our Primary Care Team. As a member of the Integrated Behavioral Health team, you will provide short-term services to assist patients of all ages and families in coping with personal, financial, psychosocial, and environmental problems associated with the patient's medical and/or psychiatric diagnosis. Requirements

• Washington State Licensed Clinical Independent Social Worker.
• Two (2) years of recent LCISW experience in a health care setting.
• Minimum two years of Social Worker experience in a healthcare setting, clinical social work in a similar setting, or case management role preferred.
• Medical social work experience preferred.
• Experience in working with specific patient populations that may include but are not limited to children, frail elderly, vulnerable adults, and patients impacted by mental illness.
• Previous experience in the AIMS collaborative care primary care clinic is a plus.
• May require a valid Washington state driver's license.
