The Medical Social Worker is a licensed social work professional who is responsible for the delivery of clinical social work services to meet the medically related psychosocial needs of patients and families served by the hospital, in a manner reflective of the department's philosophy and objectives. Provides consultation to hospital and medical personnel related to biopsychosocial issues, continuity of care and discharge planning functions.

Requirements

  • Bachelor degree in Social Work, or related degree, required.
  • Graduate degree preferred.
  • Current Licensed Certified Social Work (LCSW) license by the Massachusetts Division of Registration in Social Work or license eligible.
  • One to three years of social work experience in a health care setting, preferably in acute care.
  • Demonstrated ability in psychosocial assessment, knowledge of community resources, crisis intervention skills, ability in discharge planning, and ability to work with a multidisciplinary team.
  • Data compilation and analysis skills.
  • Excellent written and communications skills.
  • Basic computer skills.

Responsibilities

  • Demonstrates knowledge of social casework theory and practices.
  • Demonstrates knowledge of bio-psychosocial factors related to illness.
  • Demonstrates knowledge of community health and social resources
  • Demonstrates knowledge of patient rights, including confidentiality, access to care, self-determination, and provider disclosure.
  • Demonstrates knowledge of legal decision-making process & options
  • Planning--attends all Interdisciplinary Patient Care Team meetings
  • Describes behavioral symptoms and observations objectively, in logical sequence, using proper English, terminology, and approved abbreviations
  • Uses effective verbal and nonverbal communication appropriately with others
  • Informs the Director of any and all discharge planning incidents which are barriers to effective treatment planning and care delivery
  • Conducts intake interviews with patients and families to develop psychosocial evaluations.
  • Works with outpatient providers to coordinate treatment planning.
  • Documents all information obtained in evaluation, assessment, consultation, referral and disposition to comply with regulatory requirements, as well as ensuring best possible patient outcome.
  • Coordinates discharge planning which includes the assessment of housing, benefits and treatment needs. Plays key role in case management.
  • Acts as liaison with community and government agencies and develop referral network as directed.
