About The Position

Requirements

• Master's degree in Social Work or a related field.
• Certification as a Medical Social Worker (MSW).
• Clinical skills in biopsychosocial assessment and interventions.
• Ability to work effectively in a team and collaborate with others.
• Knowledge of transitions of care and community resources.
• Understanding of child, elder, and dependent adult abuse reporting requirements.
• Knowledge of behavioral health and mental health assessment skills.
• Ability to assess and develop care plans for suicidal behavior and substance use issues.
• Strong communication skills, both verbal and written.
• Proficient in using computer software for data entry and communication.

Nice To Haves

• Experience in palliative care and end-of-life discussions.
• Familiarity with bioethics and legal issues impacting patient care.

Responsibilities

• Provide biopsychosocial assessment and crisis intervention for patients and families.
• Facilitate short-term counseling and psycho-education for patients and families.
• Advocate for patients and families, linking them to community resources.
• Consult on psychosocial aspects of care as part of the interdisciplinary team.
• Participate in discharge planning and collaborate with multidisciplinary treatment teams.
• Educate staff and other hospital departments on psychosocial care.
