About The Position

We’re looking for a dedicated Social Worker to provide supportive counseling, care coordination, and psychosocial assessments for individuals facing illness and disability. The UVM Health Network - Home Health & Hospice helps people live their fullest lives by providing innovative, compassionate care wherever they call home. We offer services for all ages and stages of life, including home health nursing, physical, occupational and speech therapy, children’s and family services, as well as palliative and interdisciplinary hospice care.

Requirements

• Master’s Degree in Social Work (MSW) required
• Vermont LMSW required; LICSW preferred
• At least 2 years of relevant experience (healthcare experience preferred)
• Strong communication, counseling, and rapport-building skills
• Ability to assess psychosocial needs and explain medical information clearly
• Experience with crisis intervention, community resources, and care management
• Knowledge of Medicare/Medicaid, insurance processes, and mandated reporting
• Reliable transportation and valid driver’s license (travel required)
• Ability to work collaboratively with patients, families, and interdisciplinary teams
• Ability to lift up to 50 lbs and navigate stairs as needed

Responsibilities

• Complete comprehensive in-home psychosocial assessments
• Support patients and families in coping with illness and disability
• Coordinate care plans with nurses, providers, and community partners
• Provide crisis intervention, counseling, and education
• Document assessments and care plans according to regulatory guidelines
• Participate in team meetings, care conferences, and quality improvement activities
