Requirements

• Current Licensed Social Worker (LSW) in Minnesota required.
• Bachelor's degree in social work or Sociology.
• At least two years of experience as a Social Worker, preferably in a healthcare setting.
• American Case Management Association (ACMA) Case Manager-SW Certification required within five years.

Responsibilities

• Support patients and families in the Intensive Care Unit during challenging times.
• Collaborate with other Social Workers and an interdisciplinary team including nurses, therapists, physicians, and dieticians.
• Manage a designated patient caseload and plan care to meet patient/family transitional needs.
• Monitor the length of stay and promote community resources.
• Provide supervision for new graduates in-house or reimburse for external supervision.

Benefits

• Starting pay begins at $27.95 per hour and increases with experience.
• Pay range: $26.59 - $39.88 per hour.
