About The Position

Organization: SCAH-Sutter Care at Home - Bay Position Overview: The Medical Social Worker provides counseling and supportive services to the patient, their family, caregivers, and SCAH staff. This position assesses the social and emotional factors related to the patient's illness and evaluates the psychosocial response to treatment and level of adjustment in alignment with SCAH, mission, goals and objectives . This position interprets to the family, patient, physician, and staff, the social and emotional factors related to illness, financial and community resources.

Requirements

  • Master's: Social Work or related field OR Masters of Social Services, MSS, degree from a school accredited by the Council on Social Work Education
  • 2 years recent relevant experience
  • BLS-Basic Life Support
  • INS-Automobile Insurance
  • DL-Valid Drivers License
  • In depth understanding of the implications of illness, hospice and/or home care death and the dynamics of grief.
  • Ability to interpret a variety of data and instructions, furnished in written, oral, diagram, or schedule form.
  • Demonstrates working knowledge of resources and treatment centers including mental health and substance abuse services and facilities.
  • Possess written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people.
  • Well-developed time management and organizational skills, including the ability to prioritize assignments and work within standardized operating procedures and scientific methods to achieve objectives and meet deadlines.
  • General knowledge of computer applications, such as Microsoft Office Suite (Word, Excel and Outlook), electronic health records (EHR).
  • Prioritize assignments and work within standardized policies, procedures, and scientific methods to achieve objectives and meet deadlines.
  • Work independently, as well as be part of the team, including accomplishing multiple tasks in an environment with interruptions.
  • Identify, evaluate and resolve standard problems by selecting appropriate solutions from established options.
  • Ensure the privacy of each patient’s protected health information (PHI).
  • Build collaborate relationships with peers and other healthcare providers to achieve departmental and corporate objectives.
