Requirements

• Bachelors in Social Work.
• Two (or more) years of related experience in a health care setting (preferred).
• One year of experience in home health/hospice (preferred).
• Must possess valid driver's license.

Responsibilities

• Planning, coordination and delivery of quality medical social work services, assuring compliance with corporate standards and federal, state and local regulations.
• Provides ongoing assessment of clients, assuring an accurate evaluation of client needs.
• Develops a comprehensive care plan that addresses the social work needs.
• Works with patients and families regarding coping with illness, accessing resources, or other issues affecting the patient's health status.
• Links client/family with needed services.
