Requirements
- Active Medical Social Worker license in the state of Nevada
- Master’s degree in Social Work (MSW)
- Minimum of one (1) year of supervised post-licensure experience in a healthcare setting
- Ability to pass a background check
- Legally authorized to work in the United States
Nice To Haves
- Home health experience preferred
Responsibilities
- Conduct psychosocial assessments and implement individualized care plans
- Provide ongoing support, education, and counseling to patients and families
- Assist patients and caregivers with self-care strategies and community resources
- Coordinate services with physicians, nurses, and other care team members
- Document patient progress and provide timely reports
- Advocate for patients to ensure continuity and quality of care
