Position Summary As a Medical Social Worker, you will play a vital role in supporting patients and families in the home setting. Responsibilities include implementing patient care plans, conducting assessments, providing progress reports, assisting families with patient self-care needs, and coordinating home health services as part of an interdisciplinary team. Requirements

Active Medical Social Worker license in the state of Nevada
Master’s degree in Social Work
Minimum of one (1) year of supervised post-certification experience in a healthcare setting
Successful completion of a background check
