About The Position

As a Medical Social Worker at Carilion Clinic, you will be an integral part of the care team, contributing your professional expertise to support the holistic health of patients and their families. You will assist patients and families with personal and environmental difficulties that predispose illness or interfere with obtaining maximum benefits from medical care. Your role will involve providing psychosocial support to help patients cope with disease and life-limiting illnesses, advising families and caregivers, providing education and support, and making referrals for additional services as needed. You will conduct psychosocial assessments to identify emotional, social, and environmental strengths and problems related to health management, diagnosis, illness, treatment, and life situations. You will also formulate, develop, and implement comprehensive treatment plans, coordinate care with the healthcare team, and monitor patient progress according to measurable goals.

Requirements

  • Bachelor's Degree in social work or related degree required.
  • Social work experience in a healthcare setting preferred.
  • Experience working in Med/Surgical units, Emergency Department, Pediatrics, and Birthplace.
  • Willingness to work with substance-exposed newborns and their families.
  • Demonstrates critical thinking and creative problem-solving skills.
  • Ability to work as an integral member of a multi-skilled team.

Responsibilities

  • Provide psychosocial assessments of patients and families.
  • Utilize assessment and social work experience to plan and coordinate patient care.
  • Formulate, develop, and implement comprehensive treatment plans.
  • Coordinate care plans with the healthcare team and make recommendations for further treatment.
  • Monitor utilization of healthcare resources and collaborate with the team to address unnecessary utilization.
  • Arrange and coordinate assistance with unmet patient/family needs.
  • Advocate on behalf of the patient in crisis situations.
  • Participate in care transitions post-hospital and/or ED discharge follow-up.
  • Collaborate with other professionals to evaluate patients' medical or physical condition.
  • Refer patients or families to community resources for access to services.
  • Participate in care team pre- and post-visit case reviews.
  • Meet documentation standards for quality, timeliness, and confidentiality of medical records.
