About The Position

Provides Masters’ level clinical social work to address patient/family psychosocial adjustment to illness and post-hospital care. Provides information, referral, and consultative services to patients, families and hospital staff. Population Served: Acute medical and surgical patients including adolescent, young adult, middle-aged adults, geriatric. Units covered include the Emergency Department, medical, med-surg, ICU, Maternal-Child, NICU, Pediatrics.

Requirements

  • Master’s Degree in Social Work from accredited Social Work Program.
  • One to two (1-2) years of work experience in a skilled nursing facility, home health, hospice, and/or an acute hospital.
  • ASW License preferred.
  • Current American Heart Association BLS Certification.

Nice To Haves

  • Hospital experience preferred.

Responsibilities

  • Conducts initial assessment of new patients to evaluate social, emotional, and physical needs by applying social work theories and techniques.
  • Coordinates with interdisciplinary team to address patients’ discharge planning needs/barriers, maintaining ongoing communication with the team.
  • Coordinates home health, hospice, skilled nursing, durable medical equipment, facility placement, community agency or medical/behavioral follow-up, and transportation services as needed.
  • Interviews and evaluates patients and support persons to identify biopsychosocial factors which promote or interfere with patients’ ability to obtain the maximum benefits from treatment.
  • Provides brief counseling to assist patients and families with grief, ineffective coping, or need for additional support.
  • Provides education and appropriate support resources.
  • Cross-trains in and able to work in all units assigned to social workers.
  • Understands and supports the rights of the patient/family, facilitates conflict resolution, and engages others in problem-solving discussions.
  • Reports suspected child, adult/dependent abuse and assault to the authorities and completes required documentation and forms.
  • Facilitates appropriate planning for newborns with alternative discharge plans (Adoption, CPS, police custody, etc.).
  • Documents patient information per hospital and external regulatory requirements.
  • Participates as requested on the Hospital’s Bioethics Committee.
  • Identifies potential problems with current or new practice, peer performance, policies, functions, services or risk management issues, immediately communicates this to supervisors, and actively participates in problem solving.
  • Follows hospital-wide and departmental policies and procedures.
  • Completes complex tasks with assistance from Lead or Supervisor and in coordination with case manager and interdisciplinary team.
