About The Position

The Medical Social Worker (Case Management) plays a crucial role in providing psychosocial support to individuals and families affected by illnesses or injuries. Under the supervision of the Senior Nurse Manager, this position involves assessing patient needs, coordinating care, and collaborating with multidisciplinary teams to ensure comprehensive support and continuity of care. The role requires a strong understanding of social determinants of health and the ability to address various psychosocial needs while maintaining effective communication with patients and healthcare providers.

Requirements

  • Master's degree in Social Work from an accredited school of social work.
  • Current and valid Arizona LSW or LCSW license required.
  • Previous medical hospital social work and/or ambulatory social work experience preferred.
  • Strong verbal and written communication skills.
  • Ability to work collaboratively with healthcare teams.

  • Experience with electronic health records and healthcare software.
  • Knowledge of federal, state, and agency laws governing social work.

Responsibilities

  • Provide psychosocial assessments to determine patient needs and make appropriate referrals.
  • Evaluate patients based on their needs and limitations, addressing care gaps.
  • Maintain consistent communication with patients and providers/case managers regarding patient concerns and goals.
  • Provide therapeutic support to patients, families, and caregivers as needed.
  • Plan, coordinate, manage, and implement support to help patients adjust to illness.
  • Collaborate with multidisciplinary teams and act as a resource for community resources.
  • Document patient situations and actions for coordinated care outlined in medical care plans.
  • Follow established policies and procedures for care coordination and continuity.
  • Consult with healthcare providers regarding patient-specific clinical and social issues.
  • Participate in patient care conferences and specialty team meetings as required.
  • Maintain professional knowledge through continuing education and training.
  • Participate in data collection efforts to track outcomes.
