About The Position

As a BSW Medical Social Worker, you will be responsible for providing essential social work services to pediatric patients and their families within a healthcare setting. You will play a crucial role in addressing the psychosocial needs of pediatric patients and supporting families through the challenges of managing their child's medical and/or behavioral health condition. Working closely with the healthcare teams, you will contribute to the holistic care approach by assessing social determinants of health, providing counseling, advocacy, and connecting families with appropriate resources.

Requirements

  • Bachelor’s degree from an accredited college or university in Social Work is required
  • Demonstrated skills in systems thinking and understanding the impact Social Determinants of Health has on health care for patients and families.
  • Requires knowledge of human behavior, social problems, counseling modalities and community resources.
  • Ability to communicate effectively with patients/families, physicians, other health care professionals, attorneys, and law enforcement officials.
  • Current and valid Social Work Certification in the state of Nebraska
  • A valid driver's license and an acceptable motor vehicle record/driving record Required

Responsibilities

  • Assessment and Intervention: Conduct comprehensive psychosocial assessments of pediatric patients and their families to identify strengths, needs, and potential barriers to care. Develop and implement intervention plans tailored to address identified psychosocial concerns and promote optimal health outcomes.
  • Crisis Intervention: Provide crisis intervention and support to pediatric patients and families facing acute medical or behavioral health crises, trauma, or difficult diagnoses. Offer emotional support, coping strategies, and facilitate communication between families and healthcare teams.
  • Resource Referral and Coordination: Connect pediatric patients and families with community resources, support groups, financial assistance programs, and other social services to enhance their overall well-being and resilience. Coordinate with external agencies and organizations to facilitate continuity of care and comprehensive support.
  • Support: Offer individual, family, and group support to pediatric patients and their families to address emotional, behavioral, and adjustment difficulties related to the child's healthcare condition. Provide support in coping with grief, loss, and transitions.
  • Documentation and Evaluation: Maintain accurate and timely documentation of assessments, interventions, and case management activities in accordance with professional standards and regulatory requirements. Evaluate the effectiveness of social work interventions and participate in quality improvement initiatives.
  • Collaboration and Consultation: Collaborate with medical teams, including physicians, psychologists, nurses, therapists, and other allied health professionals, to integrate social work perspectives into the overall treatment plan. Consult with colleagues and seek supervision as needed to enhance clinical practice and ensure ethical conduct.
  • Education and Training: Provide education and training to pediatric patients, families, and healthcare professionals on topics related to child development, family dynamics, coping strategies, and accessing social services. Promote awareness of psychosocial issues impacting pediatric healthcare.
  • Responds to Social Determinant of Health needs such as food, housing, utilities, and transportation.
  • Coordinates patient arrival, triage, and registration
  • Provides patient and family with initial paperwork and screening questionnaires
  • Utilizes Electronic Medical Records
  • Collaborates with the Providers to care for the patient.
