The purpose of a Medical Scribe at Oak Street Health is to support primary care providers with clinical documentation to enable them to focus on providing exceptional patient care. Scribes assist providers throughout the patient care journey, including huddling each morning to plan for visits, joining them in the exam room to observe and document, and assisting with next steps after visits. These care team members, internally called CISs (Clinic Informatics Specialists), support accurate, specific, and timely clinical documentation. They observe and document all patient encounters in real time, becoming experts in the value-based care model, documentation, and care of chronic conditions, including ICD-10 and CPT coding. Scribes use this expertise to help providers identify and close care gaps. They receive extensive on-the-job training in clinical workflows, value-based medicine, preventative care for chronic conditions, accurate and specific documentation, population health data streams, and team-based care. A commitment of at least 1-2 years is ideal for this role.
