About The Position

The Medical Scribe at Oak Street Health plays a crucial role in supporting primary care providers by managing clinical documentation, allowing them to focus on delivering exceptional patient care. This position involves observing patient visits, documenting encounters in real-time, and assisting with clinical documentation improvement. Scribes are trained extensively in clinical workflows and value-based care, making them integral to the care team and enhancing the quality of patient interactions.

Requirements

  • Knowledge of medical terminology and common medications from a pre-medical degree or prior clinical experience.
  • Strong computer literacy and ability to learn new technical workflows.
  • Fluency in Spanish, Polish, Russian, or other languages spoken by the communities served.
  • Ability to type 70+ words per minute.
  • Ability to commit to at least 1 year in role (2+ years is ideal).
  • US work authorization.

Nice To Haves

  • Prior clinical experience, including shadowing and/or volunteering.
  • Prior scribe or transcription experience.

Responsibilities

  • Documenting patient encounters in a structured note, including the history of the present illness, assessment, plan, and physical exam.
  • Assigning appropriate CPT and ICD-10 codes.
  • Preparing After Visit Summaries.
  • Requesting and reviewing medical records to support clinical documentation improvement.
  • Consulting with providers to ensure accurate and specific documentation.
  • Placing orders and referrals as needed.
  • Supporting the care team with additional responsibilities related to clinical documentation.
