The Medical Respite Case Manager works to coordinate care and support services for homeless individuals recovering from illness or surgery in a respite care setting. The case manager ensures that clients receive the necessary medical, social, and housing services to recover in a safe environment and transition to more stable housing. This position involves close collaboration with healthcare providers, social service agencies, and housing providers.

  • Collaborate with case managers, social workers, nurses, and community partners to provide clients with well-rounded care.
  • Develop, implement, monitor, evaluate, and adjust individualized care plans focused on medical, social, and housing needs, ensuring compliance with contract services.
  • Conduct comprehensive assessments of clients' health, housing, and psychosocial needs during the admission process.
  • Coordinate transportation for medical appointments and ensure clients have access to necessary medications and treatments.
  • Act as a liaison with social service agencies to secure access to food, clothing, mental health services, and addiction support.
  • Assist clients in obtaining and maintaining benefits (such as SSDI/SSI) and other mainstream services, ensuring they meet tenancy requirements.
  • Exercise independent judgment on significant issues affecting clients' welfare.
  • Oversee the security and operational control of the facility.
  • Show patience and compassion, assisting those unable to care for themselves.
  • Provide clients with information and education on health management, medication adherence, and preventive care, including hygiene and housekeeping.
  • Coordinate daily meals and nutrition services for clients.
  • Keep detailed chronological records of events during the shift in the CareLogic system, documenting any unusual occurrences in the facility logbook and other required databases.
  • Maintain accurate records of all client interactions, care plans, and services provided.
  • Prepare statistical, census, incident, and other required reports.
  • Monitor resident and visitor activities inside and outside the facility.
  • Conduct routine facility and grounds inspections to maintain security, documenting rounds in the facility logbook.
  • Ensure residents adhere to fire and other safety regulations.
  • Participate in and obtain certifications in crisis management and first aid/CPR.
  • Promptly and professionally answer incoming calls.
  • Immediately notify on-call staff of any emergency situations.
  • Supervise health-related activities and services across the care continuum, handling referrals from insurance carriers.
  • Develop and execute discharge plans detailing follow-up medical care and housing arrangements for clients.
  • Ensure clients exit the program with the necessary resources and support for continued recovery and stability.
  • Respond to client crises, including medical emergencies, behavioral issues, and housing-related challenges, providing emotional support as needed.
  • Comply with and enforce the policies and procedures of Volunteers of America and the Transitional Housing Operations Manual.
  • Cultivate relationships within the service network to improve client care.
  • Keep homeless service networks, providers, and applications updated to facilitate optimal housing placements for clients.
  • Bachelor’s Degree is required
  • Excellent written and verbal communication skills including the ability to understand written and verbal instructions
  • Requires the flexibility to work evening and weekend hours.
  • Ability to complete billing forms and documentation
  • Experience in a residential setting is preferred.
