Requirements

• At least 1 year of experience in clinical records management (home health or hospice preferred).
• Working knowledge of healthcare regulations and information systems.
• Excellent organizational, communication, and follow‑through skills.
• Ability to work independently while supporting a collaborative team.
• Professional discretion and commitment to confidentiality.

Responsibilities

• Manage daily clinical records operations and supervise the clinical records process.
• Ensure compliance with state, federal, and regulatory requirements.
• Safeguard patient confidentiality through secure record controls and monitoring.
• Conduct post‑discharge administrative record audits and escalate issues appropriately.
• Track and follow up on unsigned physician orders to ensure timely compliance.
• Maintain and support the clinical records information system, including upgrades.
• Provide backup support to intake and front office teams as needed.
• Assist with authorized record requests and general administrative duties.
