About The Position

The Medical Provider (NP/PA) is an integral member of Alluvion Health’s medical team, delivering comprehensive primary and preventive healthcare services in a clinic-based setting. This role functions both independently and collaboratively within an integrated care model, coordinating with medical, behavioral health, dental, and support services to achieve patient-centered outcomes. Depending on licensure and scope (NP or PA), the provider will diagnose and treat acute and chronic conditions, perform exams and procedures, order and interpret diagnostics, prescribe medications, and document care in the electronic health record. Providers also contribute to quality improvement efforts and support Alluvion’s commitment to exceptional, equitable care.

Requirements

  • Strong clinical assessment, diagnostic reasoning, and treatment planning skills.
  • Knowledge of health promotion, disease prevention, and management of acute/chronic conditions.
  • Proficiency with clinical tools, medical equipment, and routine therapeutic procedures.
  • Competence with electronic medical records and medical software applications.
  • Excellent communication skills (written and verbal) with patients and interdisciplinary teams.
  • Ability to work independently, manage competing priorities, and collaborate effectively.
  • Maintains professionalism and manages conflict appropriately.
  • Strong commitment to HIPAA privacy and security rules.
  • Graduate of an accredited NP program.
  • Licensed to practice as a Nurse Practitioner in Montana with prescriptive authority (as applicable).
  • Current DEA registration (as applicable to prescribing).
  • Board certified or board eligible.
  • Graduate of an accredited PA program.
  • Licensed to practice as a Physician Assistant in Montana with prescriptive authority (as applicable).
  • Current DEA registration (as applicable to prescribing).
  • Active Montana license to practice (NP or PA, as applicable).
  • Valid Driver’s License issued by the State of Montana.

Responsibilities

  • Provide high-quality primary and preventive care across the lifespan (as appropriate to training and experience).
  • Perform exams, evaluations, assessments, diagnoses, and treatment planning.
  • Order, interpret, and follow up on laboratory tests, imaging, and other diagnostics.
  • Prescribe medications and treatment regimens within scope and regulatory requirements.
  • Educate patients and families on diagnoses, treatment options, prevention, and health maintenance.
  • Establish rapport and trust with patients to support engagement and outcomes.
  • Perform routine in-office procedures consistent with competency (e.g., wound care, suturing, therapeutic interventions).
  • Document patient histories, physical findings, assessments, plans, and follow-up in the EHR accurately and timely.
  • Maintain thorough case histories and complete follow-through on patient progress and care coordination.
  • Collaborate with physicians, nursing, care teams, and other departments to ensure coordinated, patient-centered care.
  • Participate in integrated workflows that support whole-person health.
  • Communicate clearly and respectfully with patients, peers, and internal teams.
  • Follow Alluvion Health policies, clinical protocols, and standards of practice.
  • Support quality improvement initiatives and organizational practice principles.
  • Maintain compliance with HIPAA, privacy/security standards, and applicable clinical regulations.
  • Participate in policy/protocol development and implementation as assigned.
  • Work flexible hours and overtime when requested to support clinic operations and patient needs.
  • Participate in off-hours/coverage expectations as applicable to ensure continuity of care.
