Medical Practice LPN position in the Primary Care department in Brookfield, CT. 40 hours per week, Monday-Friday, 8-hour shifts between the hours of 7:30am-7:00pm. Summary: To assist the clinician in providing medical care to the patients of the Medical Practice.

Requirements

  • Ability to travel between practices as needed.
  • Active LPN License based on state
  • BLS certification required within 30 days.
  • HS Graduate or Equivalent.
  • Graduate of an accredited LPN Program.

Responsibilities

  • Prepares materials, supplies, equipment, and the patient to assist clinician with the patient's medical exam.
  • Obtains the patient's history and medical information.
  • Provides direct patient care as directed by a Registered Nurse or clinician.
  • Collects and processes specimens as directed by the clinician and performs point of care testing.
  • Administers medications, and vaccinations as directed by the clinician. Instructs the patient and family on how to perform clinician recommended preventive care, takes prescribed medication, prepares for tests, monitors treatment progress and notes symptom changes.
  • Contacts the patient to provide requested follow-up, advises them of test results or relays clinicians instructions.
  • Fulfills and adheres to all compliance responsibilities related to the position.

  • Demonstrates regular, dependable, and predictable attendance.
  • Performs other duties as required.
