High Lakes Health Care , of Praxis Health, is seeking a motivated team-player to work as a Medical Office Support professional on our primary care behavioral health team. This person works as a care coordinator and as a provider extender that guides patients through and around barriers in the complex healthcare system. As our Medical Office Support Professional/Behavioral Health Navigator, you will serve as a linchpin in connecting patients to essential behavioral health services, removing access barriers, and amplifying the impact of our BHCs and primary care teams. You’ll directly influence patient outcomes, care continuity, and the success of our integrated care model.
