Summary: Assists the Medical Group Practices with clinical support primarily for triage, screenings, and all patients follow up.

Requirements

  • Five years job-related experience preferred.
  • Medical knowledge and terminology.
  • Computer skills/data entry.
  • CT RN License.
  • Microsft Word, Excel and Outlook).
  • Knowledge of insurance contracts and requirements.
  • Clinical and business office functions, including direct patient care.
  • Verbal and written English proficiency.
  • Minimum Experience: three years
  • Credentials: RN

Nice To Haves

  • Registered Nurse BSN preferred.
  • Previous medical office experience preferred.

Responsibilities

  • Obtains subjective and objective patient information to effectively triage and manage patient care and assure all patient follow up is implemented as outlined by the clinicians.
  • As delegated, instructs patients and their caregiver on how to perform clinician recommended preventive care, take prescribed medication, prepare for tests and procedures, monitor treatment progress and document symptom changes.
  • Assists with complex prior authorizations for medications.
  • Contacts the patient by telephone to provide requested follow-up, advise them of test results or relay a physician's instructions.
  • Communicates test results as designated by the Clinician, sending letters, setting reminders, and placing advisories when indicated.
  • Proposes medication renewals.
  • Provides patient education and support for illnesses or injuries.
  • Performs prescreening telephone visits as instructed by clinicians.
  • Performs organizational responsibilities to achieve the Practices' and NHMP missions through a process of total quality improvement.
  • Act in accordance with Nuvance service excellence standards and values.
  • Fulfills all compliance responsibilities related to the position.

  • Demonstrates regular, reliable and predictable attendance.
