This position supports the Wellness Clinic by working to help empower patients to meet their health goals and support positive behavior change towards that goal by establishing a plan of care with patients and providers that identifies and continuously reevaluates appropriate levels of care thereby enhancing communication and collaborative relationships with multidisciplinary healthcare providers . Essential Duties/Requirements/Activities: Assist with medication adherence, appointments, referral scheduling, tracking follow up results from referrals, understanding health insurance coverage, reminders, transition of care, wellness education, health support and/or lifestyle medication, and behavior changes. Provide comprehensive transitional care to include appropriate follow up from outpatient to other settings and updating care coordination documentation. Communicate with the patient, authorized family, and/or caregivers in a culturally appropriate manner regarding the assessment of care decisions, including the identification of authorized representatives. Monitor, arrange, and evaluate the effectiveness of appropriate evidence-based and/or evidence-informed preventative services. Coordinate or provide behavioral health, oral health, chronic disease management, recovery services and social health services available in the community, behavior modification interventions aimed at supporting health management (including but not limited to, obesity counseling, tobacco cessation, and health coaching, and comprehensive transitional care from inpatient to other settings, including appropriate follow-up). Assess social, educational, housing, transportation, and vocational needs that may contribute to disease and/or present as barriers to self-management so that the plan of care incorporates areas of needs, strengths, preferences, and risk factors. Maintain system and written standards and protocols for tracking eligible referrals. Ability to perform Medical Assistant duties as per process and assess and facilitate improved health literacy and self-management skills. Medical Assistant duties included but not limited to phlebotomy and vitals. Refer member to community and social support services to provide recovery and social health services, and understand eligibility for these programs, disability benefits and identifying housing programs. Ability to cover the Wellness Clinic when coverage is needed. Performing Medical Assistant duties and assisting the primary provider. Advocate for individuals and families. Assist in outbound or inbound marketing activities by demonstrating expertise in various areas. Plan and execute initiatives to reach HEDIS measures. Knowledge and demonstrated abilities to work in a regulatory climate that includes oversight by federal and state laws, and contracts with insurance companies. Effective oral and written communication skills, and ability to maintain patient confidentiality. Excellent interpersonal skills reflecting clarity and diplomacy; communicates accurately and effectively with all levels of employees and management. Detail oriented, thorough and able to handle multiple tasks and projects with varying deadlines and priorities. Ability to apply critical thinking skills and make sound judgement both while performing daily responsibilities and throughout the patients’ continuum of care. Knowledge of a Care Coordinator practice.
