About The Position

Lifespan is a trusted, mission-driven, regional nonprofit dedicated to helping older adults take on the challenges and opportunities of longer life. The Healthcare Coordinator provides one-on-one health coordination and support to older adults residing within a community-based setting within designated surrounding counties. The Healthcare Coordinator works closely with the RN Program Coordinator assisting clients with learning how to manage their healthcare as directed by medical providers, with the goal of working towards independence when possible. Healthcare coordination services would be provided to a case load of approximately 25 to 30 clients, depending on complexity of needs. This role does not involve hands on, clinical nursing.

Requirements

  • LPN License and NYS certification and one year of experience in the healthcare field is preferred OR a combination of education and two years’ experience in a community healthcare setting.
  • Minimum of one- year experience working in the health services industry or experience as a medical liaison is required.
  • Must be able to meet with clients in their homes and in a variety of community and medical office settings
  • Must have reliable transportation and valid driver’s license
  • A portion of the work is performed at a desk or at a personal computer station
  • Excellent verbal, written and telephone communication skills required
  • Computer skills required
  • Must have proven time management skills and ability to work independently under supervision
  • Ability to maintain composure under strict deadlines and difficult situations.
  • Ability to work with diverse, multi-cultural client base

Nice To Haves

  • Knowledge and experience with geriatrics, intellectual and developmental disabilities preferred.
  • Bi-lingual a plus.
  • Experience with substance use disorder a plus.

Responsibilities

  • Schedule medical appointments
  • Help identify and schedule available transportation if needed
  • Provide advocacy and support during medical appointments
  • Act as primary liaison to medical providers, families, advocates and other service providers
  • Reconciles medication with provider and client
  • Monitors outcomes of appointments and encourages completion of necessary medical follow up
  • Explains, educates, and encourages client in developing a healthier lifestyle
  • Collaborate with and refer to social work when psycho-social needs or services are identified
  • Provide home visits to assist with health education
  • Maintain accurate, timely documentation
  • Other duties as assigned by supervisor
