About The Position

The Medical ICU Nurse Practitioner at Memorial Hermann is responsible for providing comprehensive patient care in the Medical ICU, including assessment, diagnosis, treatment, and management of patients. This role involves collaboration with an interdisciplinary team to ensure high-quality care and adherence to clinical guidelines. The position requires prior experience in ICU settings and offers opportunities for professional growth and advancement through the Professional Nurse Advancement Program.

Requirements

  • Graduate of Nurse Practitioner Program approved by the State of Texas
  • Masters of Nursing preferred
  • Current State of Texas license to practice professional nursing
  • Certification by the American Nurse Credentialing Center or National Certification Center in area of specialty
  • Certification in Basic Life Support; current Advanced Cardiac Life Support (ACLS) or Pediatric Advanced Life Support (PALS) or Neonatal Resuscitation Program (NRP)
  • Certified in ATLS (Advanced Trauma Life Support) if area of practice is Trauma within 6 months of hire date
  • Two (2) years clinical nursing experience in area of intended practice

Responsibilities

  • Primarily cover the Medical ICU with overflow in Surgical ICU
  • Review the patient's charts daily and assess for any gaps in treatment
  • Pre-round before multidisciplinary rounds, assess patients and make changes to the patients plan of care
  • Order AM labs and diagnostics as necessary
  • Focus on ICU metrics including line necessity, ventilator weaning, blood glucose control, order entry, restraint utilization, core measures and home medication reconciliation
  • Assist with the documentation of problems and diagnosis
  • Document transfer notes for all patients transferring the unit
  • Assist with discharge order and documenting a discharge summary
  • Round on the unit at the beginning of the shift and place orders as needed with Intensivist consultation
  • Assist with the coordination of multidisciplinary rounds
  • Assist with new admissions by entering admission orders and brief HPI if needed
  • Assist with family communications and provide patient education
  • Perform invasive procedures such as central line placement, arterial line placement, PICC line, midline, dialysis catheter placements, and endotracheal intubations
  • Assist with the removal of drains and dressing changes
  • Respond to CODE BLUE activations in the MICU and SICU medical overflow, or as requested by the Intensivist on call for West Tower
  • Serve as a resource and provide education to nursing staff as needed
  • Participate in discharge planning, MDDR rounding with Case Management
  • Follow up on downgraded patients and ensure appropriate orders are placed
  • Post-round with Pharmacist for orders recap after rounds and at the end of the shift
  • End of shift rounds with nursing staff and Intensivist
