About The Position

The Medical & Education Coordinator provides vital administrative, educational, and medical support for the CFS Point of Contact, ensuring that children and families receive comprehensive, timely services. This role coordinates educational assessments, services, and advocacy while also managing medical documentation, compliance requirements, and healthcare follow‑up for youth in care. Additionally, the position supports senior leadership through high‑level administrative functions, data reporting, and project coordination to help drive organizational goals. The coordinator serves as a key liaison across internal teams, external providers, schools, and healthcare partners to ensure consistent communication and high-quality service delivery.

Requirements

  • Bachelor’s degree preferred. A high school diploma with 4+ years of office, medical and/or education experience may be considered in lieu of a degree.
  • Excellent organizational and interpersonal skills; demonstrated compliance with agency and regulatory mandates; the ability to work with persons from various ethnic backgrounds, with particular emphasis in working with families of African American heritage; ability to navigate and input data into the Key Information Demographics System; The candidate must demonstrate they can work autonomously and with a team using due diligence measures when needed to meet a goal
  • The candidate must possess a proficient knowledge of DPW regulations
  • The candidate must demonstrate through their actions a team-player philosophy
  • Must type a minimum of 65 WPM
  • Proficient in Microsoft Office Suite (Word, Excel, PowerPoint) and Canva
  • Excellent verbal and written communication skills
  • Strong customer service skills and a professional demeanor
  • Ability to work after hours and occasional weekends as needed
  • Ability to handle confidential and sensitive information with the highest level of professionalism and discretion

Nice To Haves

  • Knowledge of Medical and/or Educational systems strongly preferred
  • One to three years in a high-level office environment, with a minimum of one year of Human Resources experience preferred.

Responsibilities

  • Confirm Children on ASCI case load and list demographics to indicate age, sex, and educational obtainment
  • Confirm current attendance and grades of students
  • Assess student educational needs and make appropriate referrals and intervention recommendations including: Special needs education and behavior plans, Conferencing with ASCI and CYF caseworkers, Lead Education Administrators and school personnel regarding education plans After care plans and/or needed petitions for special educational placements
  • Refer students to educational and/or vocational programs, including (but not limited to): Tutoring, Mentoring, Job training, College prep/fairs, etc.
  • Coordinate and host education informational events for children/students, parents and caregivers
  • Recommend student involvement in school and community events along with working with HIU staff on the following: After school programs, Leadership/youth development activities Violence prevention/education Substance abuse prevention, etc.
  • Screen age appropriate children for educational programming and services including: Determine and conduct needed vocation or education assessments
  • Coordinate and collaborate with the Triad, Internal and External Providers to ensure the educational needs of youth are met.
  • Request, track, and obtain medical, dental, vision, and behavioral health records from healthcare providers for children open in POC.
  • Maintain accurate and up-to-date files in the agency’s database and case records.
  • Ensure medical documentation meets compliance standards for licensing, accreditation, and contractual requirements.
  • Review case-specific medical histories to identify service gaps, missed appointments, or unmet health needs.
  • Collaborate with caseworkers, caregivers, and medical professionals to schedule required evaluations, treatments, and follow-ups.
  • Monitor timelines for required health screenings, immunizations, and annual medical requirements.
  • Serve as a resource to staff, foster parents, and youth regarding medical needs and available healthcare resources.
  • Advocate for appropriate and timely medical interventions in coordination with case teams.
  • Provide education to caregivers on following medical treatment plans and accessing services.
  • Ensure compliance with state regulations regarding medical care for children in kinship foster care.
  • Prepare information for permanency, administrative reviews, internal or external audits as needed.
  • Track and ensure that profiles/profile addendums needed for permanency for youth to proceed
  • Track agency medical service outcomes and recommend improvements to processes and systems.
  • Report all information and needed advocacy to CFS representatives/designees about interactions in the above named areas
  • Develop and report weekly, monthly , quarterly and annually; Outcomes based on identified CFS Goals
  • Serve as an administrative support to the MD of Child and Family Services; also provides supportive functions for the Director(s) of Child and Family Services and Kinship Navigation as needed.
  • Types correspondences including reports and highly confidential materials. Must be able to generate high quality reports that are strategic in nature.
  • Assists in generating data reports inclusive of data analysis for monthly reports, outcomes/PBC tracking, board reports and any additional data analysis requested.
  • Assists in helping to generate data tracking mechanisms for the division. Reports include monthly CPO, Weekly Over 60, Weekly Report, Quarterly Board Report, Contract Monitor and Gold Standard.
  • Arranges luncheons, coordinates special events, and oversees special projects for CFS
  • Takes meeting minutes, types and distributes in a timely manner (within 24 hours of the meeting) and follows up on all action items.
  • Enacts project management for the division and utilizes Microsoft Planner to oversee the completion of associated deadlines.
  • Provides updates to the MD/CFS Leadership regarding status of projections and execution of associated plan.
  • Serves as a liaison for the MD or stand-in for the MD when the MD is not available to participate in meetings. Represents the MD’s office at all times in a professional manner
  • Manages the MDs calendar, calls and all meetings requests. Ensures that the MD’s schedule is organized at all times.
  • Works with the MD to enact the strategic vision for the division through collaborating inside CFS and with all supplemental departments.
  • Helps to assess gaps that are seen in the division and utilizes that information to determine processes, protocols and opportunities for improvement to meet contractual regulations.
  • Provides oversight to CFS administrative staff, on an as needed basis
  • Works collaboratively with the Executive Assistants and agency staff to complete assignments and special projects
