About The Position

The Medical Doctor will work with the CN Guidance FACT team to deliver comprehensive psychiatric services to individuals diagnosed with severe mental illness whose needs have not been adequately addressed through traditional service delivery methods. The provider will collaborate with a multidisciplinary team to ensure integrated care, using psychiatric evaluation, medication management, and treatment planning tailored to each individual's unique needs.

Requirements

  • Medical degree (MD or DO) from an accredited program, with a New York State medical license and certification by the American Board of Psychiatry and Neurology, and DEA registration.
  • Extensive clinical experience in diagnosing and treating psychiatric disorders, including severe mental illness, in both outpatient and inpatient settings.
  • Experience managing children with severe emotional disturbances (SED) and individuals with psychiatric and co-occurring substance abuse disorders.
  • Familiarity with psychiatric techniques in evaluation, medication management, and treatment planning.
  • Experience working with severely impaired patients is required.
  • Familiarity with current DSM criteria.

Responsibilities

  • Provide psychiatric and medical assessment, diagnosis, and treatment for patients with severe mental illness, including the development of individual service plans.
  • Supervise and manage medication administration, including prescribing psychopharmacological treatments, and monitoring for effectiveness and side effects.
  • Collaborate with the multidisciplinary team to provide integrated care and ensure compliance with County, State, and Federal regulations.
  • Provide clinical supervision and training to team members, ensuring the delivery of high-quality, evidence-based care.
  • Conduct evaluations and assessments on new recipients prior to FACT service admission and following hospitalization or crisis intervention.
  • Develop and maintain medication treatment plans, ensuring patients understand the effects, risks, and benefits of prescribed medications.
  • Review the appropriateness of existing medications with the team, adjusting as necessary.
  • Facilitate shared decision-making with patients and their families, ensuring they are informed of treatment progress and decisions.
  • Contribute to the timely production of all necessary reports for other agencies regarding patients' care and treatment.
  • Schedule patient contacts efficiently, maximizing time for direct patient care.
  • Be available on-call as needed, including availability for crisis intervention and emergency services on a 24/7 basis.
  • Complete QI Training during onboarding and participate in ongoing QI refresher training as required.
  • Participate in agency-wide training and work groups, particularly in relation to the Forensic ACT model and NYS-specific training.
  • Perform other duties and assignments as requested by the team leader and Medical Director.
  • Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress
  • Complete QI Training during onboarding and participate in QI refresher training as needed.
