About The Position

Multidisciplinary care teams collaborate with patients, families, and other healthcare providers to deliver preventative, proactive, and tailored care. Using an evidence-based approach, Bluestone care teams manage chronic conditions, address social determinants of health, manage transitions, and provide behavioral health support. Care teams travel directly to patients in Assisted Living, Memory Care, and Group Home communities across Minnesota, Wisconsin, and Florida. The Physician & Medical Director role is a pivotal leadership position that combines high-level population health strategy with compassionate, direct patient care. This role is responsible for driving the value-based care model, providing expert mentorship to a dedicated team of APPs, and ensuring clinical excellence for residents in assisted living and memory care environments, bridging clinical leadership and hands-on geriatric care.

Requirements

  • Medical License: Must be licensed or willing to become licensed in the state of Florida.
  • Board certification: ABMS or AOA
  • Experience: Proven experience in clinical practice, with a strong background in leadership and mentorship.
  • Skills: Excellent communication and collaboration skills, with the ability to lead and support a diverse team of APPs.
  • Commitment: A dedication to upholding high standards of clinical excellence and a passion for improving patient outcomes.

Responsibilities

  • Serve as the Primary Care Provider (PCP) for a dedicated panel of patients residing in assisted living and memory care facilities.
  • Provide comprehensive chronic and acute care management, tailoring treatment plans to the unique needs of geriatric and complex populations.
  • Actively partner and communicate with assisted living staff to ensure seamless care coordination and adherence to practice standards.
  • Partner with a team of APPs to drive population health management initiatives, specifically focusing on the success of Value-Based Care programs.
  • Provide clinical and operational mentorship to APPs, guiding them in the management of complex patients and efficient practice management.
  • Participate in key committees and task forces to lead process improvement, quality assurance, and policy development.
  • Conduct telehealth visits to satisfy state regulations and the specific regulatory requirements of various healthcare programs.
  • Provide high-level clinical support and telehealth coverage for complex patient cases or urgent acute needs across the broader clinical team.
