About The Position

The Medical Director for the House Calls Program at Oak Street Health will play a pivotal role in leading in-home primary care teams, focusing on value-based care for older adults. This position involves direct management, oversight, and coaching of local teams, while also participating in the leadership team to develop programs aimed at improving patient care and outcomes. The Medical Director will also see patients in their homes and provide strategic input on clinical models and practices.

Requirements

  • Experience as a Primary Care Provider with a passion for the geriatric population.
  • Managed care and/or value-based practice experience.
  • Ability to analyze cost, quality, and satisfaction data using a metrics-driven approach.
  • Experience in developing and implementing new processes.
  • Understanding of managed care basics and population health principles.
  • Ability to remain calm in stressful situations.
  • Commitment to older adults and low-income patients.
  • Comfort with ambiguity and problem-solving skills.
  • US work authorization.

Nice To Haves

  • Geriatric certification and/or other professional degrees (e.g., M.B.A, J.D., M.P.H).

Responsibilities

  • Lead At Home providers to align their priorities and outcomes with organizational goals.
  • Provide timely and constructive feedback to improve performance for patients and colleagues.
  • Help providers master data-driven tools to keep patients well and out of the hospital when avoidable.
  • Review quality dashboards with providers and coach on tools to improve quality outcomes.
  • Coach providers on appropriate coding and documentation.
  • Provide managed care experience and teaching to providers.
  • Participate in the At Home leadership team to enhance the care model through various divisional projects.
  • Understand and communicate provider performance and compensation plans.
  • Conduct regular performance reviews and manage human resource topics for providers.
  • Foster a culture of innovation and open dialogue within the organization.
