About The Position

Impact Health is looking for a Medical Director (Attending Physician) in the Fairfield, CT area! Medical Director who is a licensed physician (a Doctor of Medicine or Osteopathy) that organizes and assumes overall responsibility for the medical component of Skilled Nursing Facility setting. Provide clinical leadership to ensure high quality patient centered care in compliance with Federal, State, and local regulations. Partners with leadership, attending physicians and interdisciplinary team to promote overall clinical excellence with continuous quality improvement. Responsibilities: Available for consultation 24 hours per day, 7 days per week and arranges coverage when not available. This role is for Physicians who desire attending and or Medical Directorship with in Skilled Nursing Facilities. Certify that the patients meet the medical criteria for overall care based upon available diagnostic and prognostic indicators, review of medical records, and nursing reports. Collaborate with the interdisciplinary team to ensure that the medical needs of the patient are met and provide evidence-based initiatives for individual diagnosis, treatments, and plans of care. Review of medical records, and nursing reports. Review clinical outcomes, adverse events and infection control programs and Oversee Quality Assurance and QAPI. Flexible schedule for administrative duties. Maintain on-site presence and be available for consultation when clinically necessary. Requirements
