POSITION SUMMARY To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. Schedules resident appointments and arranges transportation by performing the following duties explained in the Essential Functions.

Requirements

  • High school graduate; college degree preferred.
  • Minimum one-year experience in a business setting, healthcare preferred.
  • Must be comfortable driving vehicles that seat up to 10-12 passengers and is able to manage time well.
  • Must be able to utilize Safe Defensive Driving Skills.
  • Duties include scheduling medical appointments, driving residents to these appointments and complete documentation.
  • Experience in customer service environment, preferably in a skilled nursing setting.
  • Knowledge of medical terminology helpful.
  • Proficiency in Microsoft Word, Microsoft Excel, Power Point, E-mail and the Internet.

Responsibilities

  • Schedules, documents and coordinates resident appointments and transportation.
  • Establishes and publishes daily trip schedule to care center through unit binders.
  • Completes and distributes appointment packets to include specific medical information for each resident.
  • Coordinates escorts as needed to accompany residents who require assistance.
  • Provides back up for IL Transportation Supervisor, as needed.
  • Other duties as assigned by Supervisor.
