About The Position

The Medical Coordinator at Adult Learning Learning-Lower Michigan Inc. is responsible for providing compassionate and professional care to individuals with disabilities in a 24-hour licensed residential setting. This role is crucial in supporting consumer' daily living activities, ensuring their well-being, and promoting a safe and nurturing environment. Other Hiring locations include: Sterling Heights, MI Macomb County Applicants must be available to work days and afternoons during the following shifts: 7am - 3pm (Daytime) 3pm - 11pm (Afternoons)

Requirements

  • A high school diploma or equivalent is preferred, or 1 year of experience in a health care setting providing consumer care.
  • Must be 18 years old
  • Must be able to lift 50 lbs and physically position individuals (with training). This is an essential job function that may involve 20% of scheduled time.
  • Must be able to transport consumers and have a valid driver's license. This is an essential job function that may involve 20% of scheduled time.
  • Strong interpersonal and communication skills, with the ability to engage effectively with clients, families, and team members.
  • Ability to assess consumers' needs and implement appropriate care plans.
  • Competency in basic caregiving tasks such as personal hygiene assistance, medication administration, and daily activity support.
  • Excellent problem-solving skills and the ability to respond calmly and efficiently in emergencies.
  • Ability to maintain detailed and accurate records of consumer progress and activities.
  • Familiarity with safety protocols and procedures in care environments.
  • Flexibility to work various shifts, including evenings, weekends, and holidays, as required by consumers' needs.

Nice To Haves

  • A Diploma or certificate in Medical Assisting is preferred.
  • Experience working with individuals with developmental disabilities or mental health conditions is highly desirable.
  • Proven track record of providing compassionate and effective care to adults in a residential or community setting.

Responsibilities

  • Assist consumer with personal care tasks such as bathing, dressing, grooming, and toileting while respecting their dignity and privacy.
  • Support consumers in managing their daily routines, including meal preparation, medication administration, and housekeeping duties.
  • Foster a positive and supportive atmosphere by engaging consumers in social and recreational activities tailored to their interests and abilities.
  • Monitor and document consumers' health and behavior, reporting any changes or concerns to the appropriate healthcare professionals.
  • Provide transportation and accompany consumers to appointments, community events, or other activities as needed.
  • Ensure a safe and clean environment by adhering to health and safety regulations and organizational policies.
  • Coordinating all appointments for participants including physicians, dentist, annual physicals, and other needs.
  • Creates consultation in E.M.R. and posts to site schedules.
  • Arranges and provides transportation as needed.
  • Monitors and arranges for refilling and renewal of all prescriptions on a timely basis, every 30 days for psychotropic prescriptions and every 90 days for all other prescriptions.
  • Disposes of all discontinued medications within contract agency and ALS-LM guidelines, documents accordingly.
  • Communicates with day program or school concerning the proper administration and transportation of resident’s medication.
  • Keeps the day program informed of new and increased doses of medication.
  • Arranges safe and approved preparation and transportation of medications during a leave of absence (L.O.A.).
  • Monitors the acquisition, storage and dispensing of medications, treatments, and records.
  • Assures a thirty day medication supply is provided for the resident at time of transfer to another program/living arrangement.
  • Responsible for obtaining physician forms with physician signature.
  • Maintains communication and documents with program nurse and keeps minutes to ensure adequate follow-up.
  • Obtains new medications at least one week before supply is exhausted.
  • Documents in health care chronological and progress notes, results of visits to health care provider.
  • Ensures adequate labels are on all medications.
  • Ensures 90 day Reviews and Standing Medical Orders are completed for all medications used.
  • Checks medical bills, completes billing, and fills out check requests in a timely accurate manner, attaching receipts.
  • Attends Medical Review as required by funding agency.
  • Follows MI mental health code.
  • Ensures copies of current prescriptions are kept in medical notebook, and are purged to site files.
  • Creates monthly medication sheets (MARS) and monitors for compliance.
  • Monitors acute medical conditions, relaying all information to supervisor and supports team..
  • Monitors bowel movements, menses, and weight and height charts, if part of ISP.
  • Contacts clinical team regarding any changes in participant medical status.
  • Completes DCW tasks including medication administration, meal preparation and electronic documentation.
  • Ensures Electronic Medical Record is current with: Physician consults Information releases Physicals scheduled in a timely manner Lab results Assists with updating participant Infection Control Sheets.
  • Oversees & assists in medication training for staff and consumers.
  • Must complete 4 job related trainings annually (Does not include CPR/First Aid and Quarterly Training Review).
  • Completes Annual self-appraisal 2 weeks prior to evaluation due date.
  • Payroll completed daily punches and bi-weekly approval of time card.
  • Complete annual training required to include; Contract Agency requirements, on-line training (if applicable), in-home training.
  • Checks email daily during scheduled shifts and responds within 1 business day.
  • Attends cultural diversity training yearly and completes cultural competence test.
  • Follows MI mental health code.
  • Follows all H&S/OSHA guidelines
  • Keeps confidential all information learned about participants, all program aspects, and follows HIPPA guidelines.
  • Respects and is sensitive to the cultural differences of participants and staff.
