Medical Coordinator Job Description Community Living Centers, Inc. is seeking a detail-oriented, conscientious, and dedicated individual to serve as a Medical Coordinator. The Medical Coordinator is responsible for coordinating and monitoring all resident medications, keeping accurate medical records, making all medical appointments, and transporting residents to appointments. This position requires a responsible, caring person, and patient person who loves working with people. Community Living Centers, Inc. provides support to adults that have developmental disabilities in a residential AFC environment. Our goal is to provide safe and comfortable homes that are welcomed in their communities. FULL JOB DESCRIPTION GENERAL STATEMENT OF RESPONSIBILITIES: Responsible for coordinating all medical and follow-up appointments and completing all required documentation. Provide direct hands-on care and assistance in daily living to individuals living within assigned Residential Group Home. Understand and execute the person-centered plan (PCP) and individual plan of service (IPOS).
