About The Position

The Medical Coordinator (MC) is responsible for assisting on ensuring the quality of medical services, treatment, and/or interventions provided to Unaccompanied Children (UC) in the program in collaboration with the contracted physicians. Medical Coordinator is the key monitor of patients' health through observing and assessing their records, symptoms, and reactions to treatment and care. They often have extensive interaction with the UC as well, guiding and instructing them in aftercare measures. The MC assist on the delivery of medical care with the emphasis on disease prevention and health maintenance while ensuring compliance with State licensing, Office of Refugee Resettlement (ORR), and all policies and procedures implemented by CFRC.

Requirements

  • Licensed practical or vocational nurse (LPN/LVN) or higher health-related qualification (e.g., licensed registered nurse (RN) with a bachelor’s degree).
  • Minimum of 1 year of employment in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical care.
  • Must be bilingual (English/Spanish) – read, speak, write.

Responsibilities

  • Coordinate appropriate health services, including urgent, emergent, and preventative health services, for children in care. This includes maintaining complete, confidential medical records for children, ensuring appropriate communication of health needs to case managers and clinicians, scheduling health care appointments
  • Coordinating transportation to health care appointments, ensuring qualified interpretation and translation services are in place, and serving as a liaison for community health care providers.
  • Provide support in coordinating medical, dental, and mental health services to meet the needs of the children and ensure that all new arrivals receive their initial medical assessments within 48 hours as required by ORR.
  • Coordinate the daily administrative activities, including scheduling and client medical record maintenance. Prepare client report transcription from physician record. Assure charts and/or files are recorded and purged as required.
  • Prepare treatment rooms for medical examinations and assist practitioners with medical procedures and other patient care.
  • Implement physicians’ orders, create nursing plan of care, administer medications, perform treatments, procedures and special tests, and document treatment as directed by physicians and required by company policy and local/state/federal rules and regulations. Ensure compliance with all health care regulations, including HIPAA and OSHA.
