About The Position

The Medical Case Manager ensures that all PATH adult program patients are referred to appropriate health and psychosocial services, as well as for entitlements.

Requirements

• High School or GED
• Associate or Bachelor's Degree - Highly Preferred
• At least two (2) years of experience in HIV treatment, medical case management and/or health education.
• Must have strong written and oral communication skills.
• Prior experience using an Electronic Medical Record preferred.
• Bilingual English/Spanish or English/Creole preferred
