About the position

The Medical Case Manager for Worker's Compensation at ForzaCare plays a crucial role in coordinating care for injured workers. This position involves working collaboratively with various stakeholders, including injured workers, insurance carriers, employers, and medical providers, to facilitate recovery and ensure a successful return to work. The role emphasizes communication, documentation, and the development of recovery plans, all while adhering to the company's core values of strength and care.

Responsibilities

  • Communicate with medical providers, customers, employers, attorneys, and injured workers both in person and over the phone.
  • Document actions, plans, and goals in emails and reports to customers using various forms and letters in the case management process.
  • Collaboratively develop recovery goals and a plan of care, ensuring injured workers' compliance and orchestrating medical care and return to work efforts.
  • Develop lasting relationships with customers, medical providers, and ForzaCare team members.
  • Ensure the case management work provided meets or exceeds customer requirements and positively impacts the assigned cases.
  • Attend appointments with injured workers, which may include visiting employers and injured workers at their place of employment.

Requirements

  • RN licensure and/or a Certified Rehabilitation Counselor with an associated college degree is required.
  • Must comply with licensure and/or certification requirements within the state(s) in which you practice.
  • Must maintain all licensure and/or certification requirements within the state(s) in which you practice.
  • Experience as a worker's compensation case manager is preferred.
  • Ability to travel using a valid driver's license, reliable transportation, and auto insurance.

Nice-to-haves

  • CCM (Certified Case Manager) certification (Preferred)
  • Experience in a fast-paced environment with multiple systems.
