The Medical Case Manager I will provide a range of client-centered, confidential services that link clients with health care, clinical psychosocial, and supportive services for clients living with HIV/AIDS who are identified as having challenges with accessing and maintaining adherence to health care services. This role is crucial in ensuring that clients receive the necessary support to stabilize their medical conditions and maintain their health. The Medical Case Manager will work closely with the CCHIP medical team to facilitate the linkage to and maintenance of clients to their primary medical services. Additionally, the case manager will provide treatment adherence counseling to ensure clients are prepared for and adhere to complex HIV/AIDS treatments. This position also involves connecting clients to other core services such as dental, mental health, and substance abuse treatment, as well as providing benefits counseling to assist clients in enrollment, verification, and utilization of those benefits. The role complies with the AIDS Foundation of Chicago (AFC) scopes and program requirements. Specific responsibilities include completing initial intake and assessment of needs for all new clients, developing comprehensive individualized service plans, and maintaining contact with clients to ensure medical and medication compliance. The case manager will make appropriate and timely referrals to both internal and external providers and coordinate services identified in the service plan. Regular meetings with medical providers will be necessary to mutually support clients' compliance with appointments. The case manager will also be responsible for properly transitioning clients out of Medical Case Management into other appropriate programs, adhering to the Social Services Documentation Policy for all patient encounters and forms, and maintaining a working knowledge of community and internal programs that enhance clients' ability to remain in care. Furthermore, the case manager will enhance their professional development and maintain an area of specialization by participating in all mandatory training and meetings, as well as contributing to Social Services Continuous Quality Improvement efforts. Providing excellent customer service and professionalism when interacting with clients and internal and external customers is also a key aspect of this role.
