About The Position

The Medical Case Manager for the HIV Program is responsible for coordinating medical and related services for individuals living with HIV. The role focuses on promoting patient adherence to treatment regimens and resolving barriers to accessing primary care. The Case Manager will provide both face-to-face and non-face-to-face services in various settings, including the office and community.

Requirements

• Registered Nurse or a bachelor's degree in social work, psychology, sociology, or a related field.
• Two years of experience in case management, social work, or a Community Health Worker Certification if holding a non-similar degree.

Nice To Haves

• Bilingual in Spanish, French, or Haitian Creole.

Responsibilities

• Coordinate medical and related services for HIV+ individuals.
• Promote patient adherence to treatment regimens prescribed by HIV Providers.
• Assist in resolving barriers to HIV primary care.
• Provide face-to-face and non-face-to-face services in the office and community.
