About The Position

Provide case management to clients with multiple medical and psychosocial needs. Coordinate psychosocial support services with community based social and substance abuse agencies and case managers. To increase client entry into and retention within medical care while improving each client’s overall quality of life through the development and monitoring of practical and realistic plans of care.

Requirements

  • Bachelor’s Degree in Social Work or human services field or combination of education and the experience listed.
  • Five years of experience relevant to case management in a healthcare or public welfare setting which includes the requirement of the current Ryan White Standard of Care.
  • Bilingual (English/Spanish)

Responsibilities

  • Maintain a caseload HIV+ clients consistent with requirements of the Ryan White Contract
  • Triage and assignment of incoming clients; maintain a waitlist as appropriate
  • Provide back-up coverage for other MCMs who may be out on vacation or on extended leaves of absence, when necessary. This will be done in conjunction with the Program Manager and the Physician Lead
  • Determine client eligibility for Ryan White funded services including financial assistance and budgeting
  • Conduct a comprehensive intake with particular emphasis on medical-related needs and other needs that may jeopardize a client’s optimal health outcomes consistent with the requirements of the Ryan White Contract,
  • Partner with each client to create an individualized Plan of Care that addresses the needs of each client and attempts to alleviate real and potential barriers to medical care and medical compliance.
  • Review each client’s medical status including CD4 counts, viral loads, medication changes and type and severity of symptoms
  • Connect clients to appropriate medical, supportive and entitlement services that have been identified in the plan of care
  • Advocate on behalf of clients for the removal of barriers to quality medical care
  • Maintain complete, orderly and updated client files in compliance with HIPAA regulations, Ryan White Part A procedures and State of Connecticut Ryan White MCM Standards of Care
  • Develop a working knowledge of government entitlement programs, AIDS assistance programs and community social service programs
  • Timely entry of activity into the CAREWare automated system
  • Complete program reports as necessary
  • Serve on agency committees as requested by supervisor
  • Local travel using a personal vehicle may be required
