About The Position

The Medical Care Manager will be responsible for providing comprehensive outpatient medical care to clients with chronic mental illnesses or substance abuse diagnoses, as well as transitioned aged youth. This role involves collaboration with case managers and treatment teams to ensure effective care delivery within the Orange County catchment area.

Requirements

• LPN certification
• Patient assessment skills
• Strong communication and interpersonal skills
• Understanding of HIPAA regulations

• Some psychiatric and medical experience preferred

Responsibilities

• Rooming clients
• Medication management
• Biometric health screenings
• Blood draws
• Communication with primary care physicians
• Consultation with our medical director and other treatment team members
