By joining our team, you will share in the joy of providing “care with dignity and life with hope.” Responsibilities: The role of the Medical Care Counselor is to support and provide health-related advocacy for individuals residing in our group homes for developmentally disabled adults.

Requirements

  • HS diploma or GED
  • Experience working with Developmentally Disabled Adults preferred
  • N.A. or equivalent training is helpful
  • Valid NYS Driver’s License
  • Ability to transfer and transport individuals to/from medical appointments in all types of weather, using house passenger van.
  • Detail-oriented, with well-developed organizational and interpersonal communication skills
  • Willing and able to provide compassionate, “person-centered” care

Responsibilities

  • Providing continuous protective oversight to the individual
  • Scheduling, transporting and accompanying individuals to appointments with providers; facilitating communication with providers re: individual’s health changes
  • Maintaining all health-related documentation, including lab work results, consultation reports and updated medication prescriptions
  • Communicating daily with nursing and residential management re: individual’s medical status and treatment plans
  • Managing medication inventory including prescriptions, over-the-counter medications, nutritional supplements and first aid supplies
  • Providing companionship visits to individuals that are hospitalized, as required
