By joining our team, you will share in the joy of providing “care with dignity and life with hope.” Responsibilities: The role of the Medical Care Counselor is to support and provide health-related advocacy for individuals residing in our group homes for developmentally disabled adults.
Requirements
- HS diploma or GED
- Experience working with Developmentally Disabled Adults preferred
- N.A. or equivalent training is helpful
- Valid NYS Driver’s License
- Ability to transfer and transport individuals to/from medical appointments in all types of weather, using house passenger van.
- Detail-oriented, with well-developed organizational and interpersonal communication skills
- Willing and able to provide compassionate, “person-centered” care
Responsibilities
- Providing continuous protective oversight to the individual
- Scheduling, transporting and accompanying individuals to appointments with providers; facilitating communication with providers re: individual’s health changes
- Maintaining all health-related documentation, including lab work results, consultation reports and updated medication prescriptions
- Communicating daily with nursing and residential management re: individual’s medical status and treatment plans
- Managing medication inventory including prescriptions, over-the-counter medications, nutritional supplements and first aid supplies
- Providing companionship visits to individuals that are hospitalized, as required
