About The Position

Reports to: Practice Manager or Ambulatory Practice Coordinator with input from assigned physician(s), nurses or other health care providers. Responsible for delivering the highest quality service to patients while contributing to the smooth functioning of practice operations. This role provides support to the practice for population management by tracking and monitoring individual process and outcome measures for eligible managed care and Medicare patients who have chronic diseases such as diabetes, cardiovascular disease, and depression. The Medical Assistant acts as a key resource to the practice for the management of registries of patients who have chronic diseases to ensure that they receive the required testing and medical management to promote optimal health and clinical outcomes. They review registries with PCPs to determine the next phase in patient care, facilitate appointment scheduling, telephone outreach and patient education, and coordinate the distribution of patient reminder letters on behalf of PCPs.

Requirements

  • High School diploma or GED.
  • Medical Assistant Certificate.
  • 1-3 years related work experience.
  • Basic familiarity with computers.
  • Ability to navigate at a basic level within web-based applications.
  • Ability to communicate clearly and effectively in written English with internal and external customers.
  • Ability to comprehend and converse in English to communicate effectively with medical center staff, patients, families and external customers.
  • Ability to demonstrate full working knowledge of standard concepts, practices, procedures and policies with the ability to use them in varied situations.
  • Ability to work collaboratively in small teams to improve the operations of immediate work group by offering ideas, identifying issues, and respecting team members.
  • Ability to provide a high level of customer service to patients, visitors, staff and external customers in a professional, service-oriented, respectful manner using skills in active listening and problem solving.
  • Ability to remain calm in stressful situations.
  • Competent to provide patient care to the following age groups: Neonatal:Birth to 6 months, Youth: 6 months to 16 years, Young adult: 16-30 years, Middle Age: 30 - 60 years, Elderly: 60 -.
  • Vaccination against influenza (flu).

Nice To Haves

  • Experience with provider relations/IPA or PO, and a familiarity with standard quality measures, e.g., HEDIS.
  • Familiarity with different electronic medical records, including WebOMR.

Responsibilities

  • Deliver the highest quality service to patients.

  • Review registries with PCPs to determine the next phase in patient care.
  • Facilitate appointment scheduling, telephone outreach and patient education.
  • Coordinate the distribution of patient reminder letters on behalf of PCPs.
  • Greet patients and serve as a communication resource.
  • Ask appropriate questions and solve problems.
  • Take complete and accurate information from patients in person and by phone, prioritizing and communicating the information to providers appropriately.
  • Respond quickly and courteously to phone calls and requests.
  • Seek assistance from supervisor and others appropriately.
  • Greet patients and escort to exam rooms.
  • Ensure availability of x-rays, lab slips, medical records and other pertinent info.
  • Obtain vitals, weight, blood pressure, pulse and temperature.
  • Perform EKGs, injections and/or phlebotomy.
  • Assist providers.
  • Perform departmental screening, lab tests and specimen collection.
  • Implement quality control standards.
  • Provide patients with relevant instructions for specific tests and procedures.
  • Perform or assist with routine procedures under proper supervision as directed by physician, Nurse Practitioner.
  • Schedule follow-up tests and appointments.
  • Arrange patient transport as needed.
  • Sort/distribute lab results daily to appropriate physician or nurse practitioner.
  • Maintain patient confidentiality.
  • Assist with procuring medical records.
  • Ensure patient records and relevant documents filed in the record have appropriate patient identification.
  • Add new medications to patient record in EMR system.
  • Assemble records according to BIDHC/APG policy and as standards are defined by the Joint Commission of Accreditation of Health Care Organizations (JCAHO).
  • Ensure that equipment and supplies necessary to carry out patient care activities are available and usable.
  • Ensure the environment is clean and organized.
  • Maintain an unobstructed area for patient access.
  • Clean rooms after patient usage and ensure appropriate disposal of contaminated materials.
  • Track, order and maintain supplies and equipment.
  • Restock supplies.
