Sequoia Hospice of the East Bay is growing and looking for experienced Medical Assistant to join our provider services team! The role of the mobile Medical Assistant (MA) is an instrumental position supporting the organization’s mission of providing life changing service. We do this by helping our patients to “Find Home” as they navigate the complex medical system. All Mas are responsible for providing competent, cost-effective, timely and high quality health care services that are within their scope of practice. Effectively deliver the responsibilities and associated operational responsibilities of a 24/7 mobile medical clinic. Maintain performance and productivity standards, operational efficiency, excellent customer service and best practice standards. Coordinate care for patients including arrangement for hospital admission, lab services, referral to specialist, home health or hospice, diagnostic tests, etc. Maintain compliance with the established standards of practice for MAs in state of practice. Prioritize workload and utilize work time properly and productively. Document findings and actions appropriately in the patient’s medical record. Create and update patient charts including all related reports, labs, medical records, orders, etc. Interview patients and POAs/caregivers and record responses accurately. This may include, but is not limited to, patient’s demographics and insurance information, patient’s current symptoms, medical history, risk factors, relevant concerns, medication lists, list of current and past Providers, etc. Schedule and coordinate for patient visits and prepare Provider notes prior to visit date. Assist with completion of forms needed by patient, insurance, community partners and Providers. Authorize medication refills as directed by Provider. Maintain patient confidentiality at all times. Participate in professional development activities and maintain professional licenses/certificates. Maintain safe work environment for self, patient, family and staff by using appropriate PPE. Work collaboratively with agency team members and community partners such as ALF and SNF staff members, patient family members and home health and hospice team members. Serve as a communication liaison between the patient, referral sources, and the licensed clinician. Represent agency in a professional manner and strive to promote a cooperative atmosphere at all times by acting with tact and diplomacy and maintaining a positive attitude. Maintain professionalism at all times by being dependable, autonomous and responsible.
