Requirements
- Medical Assistant Diploma/Certificate required

- Previous experience in senior care, home health, or primary care preferred
Responsibilities
- Support physicians, nurse practitioners, and care teams in daily patient coordination.
- Conduct patient assessments and assist in developing care plans.
- Manage referrals, follow-ups, and communication between patients, providers, and families.
- Handle inbound and outbound patient calls with professionalism and empathy.
- Maintain accurate documentation and electronic health records (EHR).
- Collaborate with healthcare teams to improve workflow and patient outcomes.
- Provide education and guidance to patients and caregivers.
- Perform other duties as needed to support patient care and office operations.
