About The Position

You’re a team-first clinician who loves making care feel seamless. In this center-based role, you’ll deliver hands-on support that keeps our PACE participants safe, seen, and supported—think vitals, labs, phlebotomy, EKGs, immunizations, wound-care support, and more within your scope. You’ll collaborate closely with our Interdisciplinary Team (IDT), surface changes in participant status, and document meticulously in the EMR to ensure every next step is clear and coordinated.

Requirements

  • Active California Medical Assistant license.
  • Phlebotomy certification.
  • High school diploma or equivalent.
  • 2+ years as a Medical Assistant in a clinical setting (PACE/geriatric experience preferred).
  • Current CPR and First Aid (or ability to obtain within 60 days).

Responsibilities

  • Perform participant intake, fulfill orders, and manage clinic flow for a smooth, efficient experience.
  • Collaborate with clinic teammates and the IDT to elevate care and the participant experience.
  • Room participants promptly and capture routine vitals and required screenings.
  • Perform labs and testing, then ensure participants get results and next-step plans.
  • Track referrals, process authorizations, clean/stock rooms, and maintain an organized clinic.
  • Assist the Clinic Supervisor with inventory, equipment, and supply management.
  • Maintain routine clinic logs and confirm equipment works as intended.
  • Ready participants for exams and support immunization prep/administration per guidelines.
  • Coordinate follow-ups, external appointments, and annual/bi-annual assessments for continuity of care.
  • Record interactions, assessments, and care plans in the EMR (e.g., PACElogic).
  • Reinforce care instructions, preventive health tips, and chronic disease management education.
  • Follow HIPAA to the letter to protect privacy—always.
  • Maintain a neat, professional appearance consistent with dress code/uniform policy.
  • Ensure clear, compassionate communication with participants and colleagues.
  • Anticipate issues (equipment, scheduling, etc.) and address them proactively.
  • Adhere to PACE, CMS, DHCS, and organizational policies without compromise.
  • Adapt to changing needs and schedules with positivity and poise.
  • Take on additional responsibilities to support the clinic and PACE program.
