About The Position

Family Health Centers of San Diego (FHCSD) is passionate about providing exceptional health care to all, especially underserved communities with limited health care options. Job Roles Using knowledge and experience as a Medical Assistant, perform telephone patient engagement as assigned by the manager using clear protocol on all calls that are transferred. Assess level of patients' understanding of care and assist in navigating FHCSD programs and services. Assist with transitional care activities for FHCSD patients. Collaborate with team members in supporting the population health management functions and interventions. Schedule appointments for patients who require more than basic information. Educate patients on the need for follow-up care. Establish Primary Care Physician (PCP), and educate on the importance of continuity of care in using PCP. Reinforce patient/family education provided to FHCSD patients. Attend staff meetings and in-services or complete make-up assignments. Follow FHCSD policies and procedures to ensure positive patient outcomes. Perform other duties as assigned.

Requirements

  • A Medical Assistant diploma from an approved course required.
  • High School Graduate (or GED equivalent) required.
  • Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.
  • Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, or Advanced Trauma Life Support, or Advance Resuscitation Training required.
  • Traveling between sites and other locations is occasionally required. Must have a valid California driver's license, an automobile, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended. An acceptable driving record is also required. California law requires all drivers to obtain a valid California driver's license within ten days of establishing residency. Reasonable accommodation may be provided on a case-by-case basis. Mileage and other reimbursement governed by policy.

Nice To Haves

  • At least 1 year of experience in ambulatory care setting or similar setting is highly desirable.
  • Bilingual in English/ Spanish may be required.
  • Ability to display cultural sensitivity and respect to ethnically and culturally diverse patient populations.
