About the position

The Medical Assistant- Care Coordination is responsible for performing administrative and clerical duties in support of the Care Coordination Department. In this role the Medical Assistant - Care Coordination is responsible for providing intermediate level administrative and clerical processing and support for the department, team members, and its varied operations, at a location assigned by management. The incumbent in this position is expected to demonstrate a proficient understanding of work processes, procedures, and program requirements to effectively support the assigned program area. Assisting with or completing these administrative and/or clerical duties, tasks, or assignments follows well-established procedures requiring some independent judgment within the scope of authority and responsibility for the position. A Medical Assistant - Care Coordination works with general supervision and review, and any work problems involving departures from standard policies, interpretations, or procedures are reviewed with the supervisor for resolution.

Responsibilities

  • Performs all assigned projects, tasks, duties, responsibilities, and assignments supporting departmental operations within established time frames.
  • Understands and complies with all organizational Code of Conduct standards, Policies and Procedures, and Service Excellence requirements.
  • Maintains patient confidentiality by controlling the environment and information being disclosed to authorized individuals.
  • Supports the delivery of comprehensive patient care and coordination of services for the patient.
  • Performs outgoing calls, contacting and communicating with patients and program participants.
  • Assists and supports the discharge care coordination team.
  • Completes administrative and clerical tasks supporting the distribution of reports and correspondence.
  • Performs data entry into program spreadsheets or database systems.
  • Demonstrates an exceptional level of customer service.
  • Maintains a clean, safe, and hygienic work environment.
  • Demonstrates understanding of Joint Commission Accreditation standards.
  • Develops and maintains positive relationships with patients, family members, and clinical staff.
  • Serves as an effective communication link between the patient and discharge care coordination staff.
  • Receives and responds to calls from patients in a timely manner.

Requirements

  • High School Diploma or G.E.D.
  • Medical Assistant Certificate from an accredited school.
  • Minimum of one (1) years' experience as a Medical Assistant.
  • Minimum of one (1) years' experience in administrative support in a clinical or medical setting.

Nice-to-haves

  • Bilingual (English/Spanish) with the ability to speak, read and write in both languages.
